Laboratory and diagnostic services, health-monitoring concierge and non-DOT panels are priced per arrangement — ask us for a quote. A $10 late fee applies after a 20-minute grace period.
Insurance
Some laboratory and diagnostic services may be billable to insurance through our laboratory partners. Coverage depends on your plan, the ordering provider and the specific test.
Eligibility verification disclaimer: being listed as an accepted plan does not guarantee coverage, payment or a specific patient cost. Benefits are verified before service where possible, and any patient responsibility is confirmed with you.
Payment & appointments
Most individual services are paid at the time of service. Employer and clinic accounts can be invoiced under an agreed arrangement.
Please arrive on time — there is a 20-minute grace period, after which a $10 late fee applies or the appointment is rescheduled.
