• Any paperwork, authorisation code, case number or donor pass the requesting employer, court or agency gave you.
• For DOT testing — the employer name, so the correct forms are used.
• For fingerprinting — the agency's card, ORI or instructions if they supplied one.
• For DNA testing involving a minor — additional documentation applies, so call us first.

There is a 20-minute grace period. If you arrive after that, either a $10 late fee applies or the appointment has to be rescheduled, depending on what the day's schedule allows.

To reschedule, call as early as you can. Mobile appointments involve scheduled travel, so they need as much notice as possible.

It depends on the role. DOT testing applies to safety-sensitive positions in federally regulated industries and follows federally prescribed procedures and forms. Everything else is non-DOT, where the panel can be tailored to the employer's own policy.

It depends on the method and the partner laboratory. A rapid screen gives a preliminary result during your visit. Laboratory-confirmed results take longer, and confirmation testing on a non-negative screen adds time.

We give you an expected timeframe when you book and follow up if it changes. Turnaround times are not guaranteed and vary with laboratory volume and the specific test ordered.

Yes — mobile collections at job sites, offices, clinics and treatment facilities, including group collections scheduled around your shifts.

Travel fees are $0.75 per mile within 30 miles of our Greenville office and $100 for locations 30 miles or more away, in addition to the service price and subject to confirmation and availability.

Yes. Individuals can book personal drug testing directly with us. No employer, referral or account is required.

A DOT drug test is $75 and a rapid drug test is $50. A breath alcohol test is $65, a five-panel hair follicle test is $200, and collections only is $50 if you have your own forms and laboratory account.

Mobile travel is charged on top: $0.75 per mile within 30 miles of our office, or $100 at 30 miles or more.

Yes, as one arrangement rather than two. A DOT physical is $150 and takes about 45 minutes. Elite1 handles the drug screen, and the examination is performed by FNP Jessica Turner, located about seven minutes from our Greenville office.

Yes. Our C/TPA and consortium service is $150 per year and covers random testing-pool management, your certificate of enrollment and DOT compliance management. We manage the pool, run selections and notify you when someone is drawn.

We provide Clearinghouse account assistance for $50, including help setting up an account and designating a C/TPA where that applies to you. We also assist with USDOT registration paperwork.

Elite1 provides administrative and coordination services. We do not determine your regulatory obligations or act as your Medical Review Officer, and responsibility for compliance remains with the employer or operator. Please confirm your obligations with your own qualified advisor.

Yes. Through our collection-site partners we can coordinate collections in other states so a required test does not mean bringing someone back to Greenville.

Employer and clinic accounts can be invoiced under an agreed arrangement.

Being listed as an accepted plan does not guarantee coverage, payment or a particular patient cost. Benefits are verified in advance where possible and any expected patient responsibility is confirmed before service.

Yes. We offer scheduled specimen pickup and shipping coordination, and on-site collection at your facility, so patients and residents do not need to be transported.

Legal DNA testing verifies identity and collects the sample under documented chain-of-custody handling, so the result can be presented to a court or agency.

Non-legal (peace-of-mind) testing uses the same laboratory analysis without that documentation, so it is generally not accepted for legal purposes.

Requirements vary by court and agency. Confirm what your specific matter requires before booking — choosing the wrong option usually means testing twice.

Yes — Fieldprint live-scan fingerprinting, alongside traditional ink fingerprinting for agencies that require a physical card. We also provide I-9 employment verification.

Nationwide and state background checks, coordinated through our screening partners, for employers, volunteer organizations and individuals.

Background screening for employment purposes is regulated. Employers are responsible for obtaining proper authorisation, providing required disclosures and following applicable federal and state requirements.

That depends on who ordered the test. Employer-ordered testing is returned through the process that program requires. Testing you arrange for yourself is returned to you.

Personal and health-related information is treated as confidential. It is collected only where needed to arrange and complete a requested service, shared only with the parties involved in delivering it, and never sold.
